The Sample Company has invested significant amounts of time and financial resources to provide a group insurance
benefit package with a goal of improving the health and welfare of its employees and their families.
This web site will describe briefly each of the group insurance benefits you will be offered for the plan year 2009 as well as
the cost per pay period for each benefit. The information provided in this document is in summary form only; you should
refer to the Certificates of Coverage provided to you by the insurance company for each of the benefit plans you enroll in
for the exact benefit coverage. The cost of Medical benefits is shared by the employees and The Sample Company, with
The Sample Company picking up the majority of the expense. The cost of the Basic Life and Disability Insurance is paid
100% by The Sample Company. The cost of the Dental, Voluntary Life and AFLAC supplemental insurance benefits is
paid 100% by the employee. Participation in the Flexible Spending Account and the 401K is voluntary. All full time
qualified employees become eligible for group insurance benefits at the end of their introductory period, which generally is
90 days. The effective date of the insurance plans the 1st of the month following 90 days of employment. Dependents are
eligible to enroll when the employee becomes eligible. Enrollment in any of the benefit plans is Voluntary. If you decide to
enroll in any or all of the plans you must submit an application for each of the benefit plans you want within the time frame
determined by The Sample Company. If you choose to not enroll in the plans, you need to fill out the Decline or Waiver
section of the enrollment forms and return them to Human Resources.
